Since stock transfers do not always indicate their source, we request that you notify us personally when making a gift of stock. Contact Amy Komarnicki at akomarnicki@ccsafoundation.org with the name of the security, number of shares, date of the gift, and if your gift is restricted to a specific program.

Donating your life insurance to CCSA Foundation may result in an attractive tax deduction. To do so, fill out a Change of Beneficiary Form available from your plan or policy provider.
